Department of Corrections and Community Supervision must assist incarcerated individuals eligible for community supervision in securing housing through comprehensive discharge planning prior to release.

Department must explore all types of housing resources available and potentially available to incarcerated individuals as part of discharge planning.

When discharge planning results in release to temporary shelter housing regulated under 18 NYCRR Parts 491 and 900, the department must reimburse the local social services district for shelter costs.

Department must provide the local social services district with a report 30 days prior to discharge describing all discharge planning undertaken, and monthly reports after discharge until the individual is no longer subject to parole.

Monthly reports following discharge must detail continuing assistance the department provides to obtain permanent housing that is not temporary shelter and complies with parole terms.
