Authorizes New York State residents to request distinctive "Alzheimer's Awareness" license plates designed and approved by the commissioner of motor vehicles.

Charges an additional annual service fee of $25 per plate, with amounts collected up to $6,000 deposited to the Department of Motor Vehicles distinctive plate development fund for production, design, advertising and marketing.

Directs service charge revenues exceeding $6,000 annually to the Alzheimer's disease assistance fund established under state finance law.

Requires a $6,000 surety bond before plate production unless the department receives at least 200 advance orders with payment; bond is discontinued if 200+ plates are sold within two years.

Takes effect 180 days after becoming law, with regulatory implementation authority effective immediately.
