Establishes a two-year demonstration program for short-term aftercare services for at-risk older adults exiting elder abuse shelters in Bronx County, New York City.

Requires the program to provide case management services, financial counseling, and mental health services beginning at discharge from the shelter facility.

Mandates evaluation of program effectiveness in preventing re-victimization, avoiding shelter readmissions, reducing hospitalizations, and maintaining community residence.

Requires submission of findings report to the director, governor, and legislature within 180 days after the two-year program concludes, including recommendations for statewide expansion and permanent program establishment.

Program takes effect 90 days after enactment and automatically expires and repeals two years after that date.
