Establishes a uniform ambulance assessment fee on ambulance services at a rate of at least 5.75% of annual revenue, with a maximum set by the commissioner not to exceed federal limits under 42 C.F.R. 433.68(f).

Exempts municipal fire departments, police departments, other government entities providing emergency medical services, aircraft-only emergency medical providers, and providers already paying assessments under article 28 of the Public Health Law.

Requires all collected fees to be deposited into a new "statewide ambulance fund" and expended solely to increase Medicaid payment rates for ambulance services and cover reasonable administrative expenses, without reducing existing state payment commitments.

Conditions implementation on federal approval from the Centers for Medicare and Medicaid Services authorizing enhanced Medicaid payments, with refund provisions if federal approval is rescinded or collection becomes impossible.

Requires the Department of Health to submit necessary documentation for federal approval, including amendments to the state Medicaid plan, within 180 days of the act's effective date.
