Candidates must submit a sworn, notarized statement within 15 days of filing nominating papers, certifying their military service record, employment history for the previous five years, current residential address, residency requirements, and educational history.

The statement must include any educational credentials or employment history that the candidate has voluntarily represented on their website, in campaign literature, or verbally during their campaign.

Boards of elections must post all filed statements on their websites within five business days and publicly identify candidates who fail to submit required statements.

Candidates must file amended statements whenever they make additional representations during their campaign, with amendments due on the date of their next campaign finance filing.

Failure to file the required statement results in civil penalties of up to $1,000 initially, plus $25 per day for statements more than 10 business days overdue, up to an additional $1,000 maximum, paid directly by the candidate and not from campaign funds.
