Establishes a process for individuals without required documentation to obtain a free non-driver identification card from the Department of Motor Vehicles by submitting a signed affidavit and birth certificate no later than seven days before an election.

Requires county, city, town, and village clerks and the Department of Health to provide certified birth certificates at no cost when presented with an affidavit signed seven or more days before an election.

Prohibits notaries from charging fees for administering oaths on affidavits required for obtaining identification cards for voting purposes.
