Requires all candidates for public office to file sworn statements disclosing their educational background (schools attended and years of attendance/graduation), military service history (years served, rank, and completion date), and employment history (positions held since age 21 or past 20 years, whichever is shorter).

Statements must be filed by the candidate personally and cannot be filed by designees or committees; false statements are punishable as a class A misdemeanor.

Establishes penalties of up to $1,000 fine or one year imprisonment for knowingly and willfully violating disclosure requirements or providing false information.

Imposes additional civil penalty up to $10,000 for three or more violations within a single election cycle for the same term of office.

Takes effect one year after enactment and applies to candidates running in elections on and after the effective date.
