Establishes a new state funding program to enhance supports and services for students with disabilities enrolled in degree-granting institutions in New York.

Allocates funds through grants to SUNY and CUNY institutions, and contracts with other tuition assistance program-funded colleges, based on proportional per-student funding for each identified student with a disability.

Permits funded institutions to use moneys for disability support accommodations, college preparation transition programs, faculty and staff disability training, and improved identification and data collection processes for students with disabilities.

Requires eligible institutions to submit applications to the Commissioner by May 1st each year demonstrating funding need and the number of students with disabilities to be served.

Specifies that funds cannot be used to support regular academic programs and must be awarded directly to institutions, not through intermediary entities.
