Amends Public Health Law Section 1356 and Agriculture and Markets Law Section 199-f to expand food allergy notices to include food intolerances in food service establishments and online ordering services.

Requires the Department of Health to provide notices on department websites in English and the twelve most common non-English languages spoken by limited-English proficient individuals, based on U.S. Census Bureau data.

Mandates food service establishments post notices in conspicuous locations accessible to food preparation and service employees, and include allergy or intolerance notifications on all menus (physical and online).

Requires online menus to provide customers either a method to indicate food allergies or intolerances, or information about allergens and intolerances for each prepared food item.

Establishes civil penalty of up to $125 per violation for food service establishments that fail to comply with the notice and posting requirements.
