Authorizes New York residents to request distinctive "epilepsy awareness" license plates designed by the commissioner of motor vehicles, issued with regular registration fees plus an additional $25 annual service charge.

Establishes the "epilepsy awareness fund" in the state finance law to receive all $25 annual service charges from the license plates, with $10,000 allocated to the department of motor vehicles one year after enactment to offset production costs.

Creates an epilepsy research and education program within the department of health to promote scientific investigations into epilepsy causes and prevention, and develop patient education programs on seizure recognition and first aid.

Requires a $6,000 surety bond for plate design and production, or waives the bond if at least 200 orders are received with payment before production begins; bond requirements expire if 200+ plates are sold within two years.

Takes effect 90 days after enactment, with immediate authorization for necessary rule and regulation amendments.
