Establishes a PFAS removal treatment installation grant program providing one-time grants of up to $5,000 for private well users installing PFAS treatment or up to $10,000 for service connection to a public water system.

Makes eligible applicants include residential property owners, tenants, and occupants, with installers authorized to apply on behalf of eligible applicants; excludes applicants with access to alternate water sources including bottled water or treatment.

Requires applicants to submit certified laboratory results showing PFAS levels exceed maximum contaminant levels, cost estimates for installation, and equipment specification sheets.

Establishes a PFAS removal treatment maintenance rebate program providing rebates capped at $1,500 for maintenance of PFAS treatment equipment installed by private well users.

Directs the department to publish program information on its website, create public education materials, distribute materials to local governments and community organizations, and compile a vendor list for treatment technology and service connections.
