Requires electronic health record vendors to integrate components allowing patients to register in the "donate life registry" for organ, eye and tissue donations in hospital systems regulated under article twenty-eight of the public health law.

Electronic health record vendors must provide this integration at no additional cost as part of existing or new implementations of their products.

Adds electronic health records as an official method for individuals to register consent for anatomical gifts, alongside driver's licenses, voter registration, health insurance applications, and other existing registration channels.

Makes registration available to individuals aged sixteen and seventeen through electronic health records and other specified methods.

Takes effect January 1 following enactment, with the commissioner of health authorized to take immediate actions necessary for implementation.
