Authorizes New York State and municipalities to establish permanent COVID-19 memorials in collaboration with municipal parks departments and the office of parks, recreation, and historic preservation.

Requires memorial designs and locations to be selected through a community-driven process with input from family members and loved ones of COVID-19 victims, with financial oversight from the comptroller's office.

Establishes the COVID-19 permanent memorial fund in joint custody of the state comptroller and the commissioner of parks, recreation and historic preservation to fund the establishment and maintenance of memorials.

Allows the fund to receive appropriations, grants, gifts, bequests, and other moneys, with income from investments added back to the fund for memorial purposes.

Authorizes municipalities to seek funding from the memorial fund as well as other public and private sources including grants, donations, and partnerships.
