Prohibits health care providers from performing sterilization procedures based solely on substituted consent from a guardian, health care proxy, or third party without the informed consent of the person being sterilized.

Allows sterilization procedures necessary to preserve life or prevent serious physical health impairment without individual informed consent.

Permits persons with disabilities to voluntarily enter supported decision-making agreements where a supporter assists with understanding sterilization information and risks, but does not make the decision on behalf of the individual.

Requires health care providers to furnish accommodations and information in accessible formats (plain language, pictures, videos, demonstrations) to enable persons with disabilities to understand sterilization procedures and make their own decisions.

Prohibits health care providers from presuming a person with a disability lacks capacity to consent based solely on their disability diagnosis, use of supported decision-making, guardianship status, or incapacity determinations in other areas.
