Defines small farms as those with annual gross cash farm income under $350,000 and midsize farms as those with income between $350,000 and $999,999.

Requires the department to provide grants, develop best practices for land transitions, and offer technical assistance through qualified providers to eligible farmers.

Mandates grant applicants submit descriptions of current crops, land to be transitioned, a transition plan using departmental best practices, projected timeline, and other information the department requires.

Requires grant recipients to provide reports demonstrating progress in transitioning to plant-based agriculture in consultation with a technical assistance provider.
