Creates a new procedure allowing voters experiencing a medical exigency (including hospitalization) that manifests no earlier than the day before an election to apply for a special ballot on election day itself.

Requires board of elections to approve the application and provide a ballot to the voter's designated agent at any time prior to polls closing, ensuring sufficient time for the voter to complete and return the ballot.

Establishes that ballots are valid if timely postmarked and must be canvassed according to section 9-209 of the election law.

Takes effect on January 1st following the date the act becomes law.
