Requires facilities to develop and update safety and security plans specifying risk reduction methods including staff training, engineering controls, alarms, barriers, and facility modifications

General hospitals in cities/counties with 1 million+ population must have at least one off-duty law enforcement officer or trained security personnel present in the emergency department at all times

General hospitals in areas with under 1 million population must have security personnel on premises prioritizing emergency department coverage, with exemptions for critical access hospitals, sole community hospitals, and rural emergency hospitals unless violence rates increase
