Requires the Department of Health Commissioner to conduct an 18-month study on using frailty index methodology to improve nursing home care quality and better direct Medicaid/Medicare payments based on quality performance

Participating nursing homes must have at least 70% occupancy, with equal representation from each department region and at least 20% being non-profit or public facilities

Divides participants into control and intervention groups, with the intervention group implementing frailty index methodology after receiving training from a contracted demonstration team

Mandates the demonstration team to train nursing home staff (nurses, aides, physicians, therapists, dietary staff, social workers, managers), collect minimum data set data, and track resident outcomes including hospital transfers, falls, pressure ulcers, and polypharmacy use

Requires reports at 14 months (year-one results) and 21 months (final report with recommendations) to the Governor and legislative leadership; takes effect September 1, 2025 and expires after 2 years
