Establishes a pilot program allowing Albany and Schenectady counties to operate self-funded or self-insured health plans that municipalities within those counties may join with mutual consent

Requires participating plans to maintain a minimum 12% reserve fund of total annual incurred claims and a 5% surplus account for unexpected obligations in case of plan termination

Mandates stop-loss insurance with a $200,000 to $250,000 deductible per individual claim and requires a minimum of 1,000 covered lives (excluding dependents)

Abolishes joint and several liability for participating municipalities, replacing it with a pro-rata assessment system based on premium equivalents if the plan becomes financially impaired

Requires annual certification to the Superintendent of Financial Services and a report to the governor and legislature 30 months after the program takes effect
