Authorizes New York State and municipalities to establish permanent COVID-19 memorials within their jurisdictions, with guidance from the Office of Parks, Recreation, and Historic Preservation

Creates the "COVID-19 Permanent Memorial Fund" held jointly by the State Comptroller and Commissioner of Parks, Recreation and Historic Preservation

Fund will consist of state appropriations, grants, gifts, and bequests, with moneys restricted exclusively to establishing and maintaining COVID-19 memorials

Memorial designs and locations must be selected through a community-driven process that prioritizes input from family members and loved ones of COVID-19 victims

Municipalities may apply for grants from the fund and seek additional public and private funding sources for memorial projects
