Expands the definition of "infertility" to include a person's inability to reproduce as a single individual or with their partner without medical intervention, and diagnoses based on a physician's findings related to medical, sexual, or reproductive history

Changes IVF coverage from three "cycles" to three completed oocyte retrievals with unlimited embryo transfers from fresh or frozen oocytes or embryos from those covered retrievals

Requires IVF procedures to follow American Society for Reproductive Medicine guidelines, including single embryo transfer (SET) when recommended and medically appropriate

Prohibits insurance coverage denial based on a covered individual's participation in fertility services provided by or to a third party (such as surrogacy or donor arrangements)

Applies to large group health insurance policies and contracts that provide medical or major medical coverage in New York State
