Expands the hospital-home care-physician collaboration program to include emergency medical services (EMS) as partners, requiring community paramedicine initiatives to include hospital, home care, physician, and EMS partners at minimum

Defines "emergency medical services" as ambulance services or advanced life support first response services staffed by emergency medical technicians (EMTs) or advanced EMTs certified under Article 30 of public health law

Authorizes community paramedicine models where EMTs and advanced EMTs can provide collaborative services in community settings beyond traditional emergency response, including services to prevent emergencies, avoidable ER visits, unnecessary transports, and preventable hospital admissions/readmissions

Requires community paramedicine services for individuals with behavioral health conditions or developmental/intellectual disabilities to include collaboration with appropriate behavioral health providers licensed under mental hygiene law

Adds orientation and collaborative training of EMS, hospital, home care, physician, and other participating provider staff in community paramedicine to the list of authorized training initiatives
