Expands continuity of care protections from hospitals only to any health care provider when contracts with health plans are terminated or not renewed

Extends the mandatory contract continuation period from two months to 120 days, during which both parties must honor original contract terms including reimbursement rates

Requires health plans to notify all affected enrollees within 15 days after the 120-day continuation period begins

Allows the Commissioner to waive the 120-day period if either party requests it when a contract is terminated for cause

Exempts situations where both parties mutually agree in writing to termination and the plan provides enrollees at least 30 days advance notice
