Establishes the New York Long Term Care Trust Program to provide long term care services and supports to eligible beneficiaries regardless of income or resources, funded through mandatory payroll premium contributions from employees and self-employed individuals

Eligible beneficiaries must be 18+, have paid premiums for either 10 years total, 3 of the last 6 years, or at least 1 year if age 55+ before program start, and require assistance with at least 2 activities of daily living (or supervision for dementia/Alzheimer's patients)

Benefits include a lifetime limit of 365 benefit units at an initial daily maximum of $200, covering services such as home care, adult day services, respite care, nursing home services, assisted living, transportation, and home-delivered meals

Creates a 15-member Long Term Care Trust Commission and advisory panel to oversee program implementation, set premium rates, adjust benefit amounts annually, and ensure fund solvency, with administrative expenses capped at 7.5% of expenditures

Allows exemptions from premium contributions for individuals with private long term care insurance, veterans with 70%+ service-connected disability, military spouses, temporary visa workers, and non-residents, though exempted individuals forfeit program benefits
