Adds nursing homes and facilities providing long-term nursing or medical services to formerly incarcerated individuals to the list of specialized facilities receiving enhanced Medicaid reimbursement rates

Guarantees these facilities receive rates no less than those in effect on December 31, 2006, adjusted forward for inflation

Applies the same rate protection to discrete units within larger facilities that specifically serve people returning from incarceration

Places post-incarceration care facilities in the same reimbursement category as AIDS facilities, traumatic brain injury units, ventilator-dependent units, behavioral intervention programs, and pediatric care facilities

Takes effect one year after becoming law
