Requires health maintenance organizations (HMOs) to offer out-of-network coverage as an optional rider to any contract and to provide at least one contract option that includes out-of-network coverage

Mandates that health insurers subject to Insurance Law Article 32 offer out-of-network coverage as an optional rider to any policy, with at least one policy option including such coverage

Extends the same out-of-network coverage requirements to health insurance corporations under Insurance Law Article 43

Requires all out-of-network coverage options to be available both within and outside the New York State Health Benefit Exchange

Takes effect January 1 following enactment and applies to contracts and policies issued, renewed, modified, or amended on or after that date
