Expands New York's 1332 State Innovation Program to provide health insurance coverage to individuals who are ineligible for the Basic Health Program due to immigration status, with household incomes up to 250% of the federal poverty line

Changes the Commissioner of Health's authority from permissive ("may") to mandatory ("shall") in taking actions to obtain federal waivers, including seeking amendments to existing waivers

Establishes enrollment prioritization for undocumented individuals, starting with the lowest income bands first, while also prioritizing those with life-threatening conditions, organ transplant needs, or serious mental illness/substance use disorders

Authorizes the Commissioner to limit enrollment of immigration-ineligible individuals to ensure program costs do not exceed available funds in the 1332 State Innovation Program Fund

Requires annual reporting to the legislature on enrollment limitations, fund transfers, and savings achieved from covering individuals previously ineligible due to immigration status
