Requires mental health reentry services to be included in every incarcerated individual's transitional accountability plan, beginning within the first week of admission to a correctional facility

Mandates mental health and trauma screenings within 60 days of commitment, with services provided by licensed therapists, psychiatrists, psychologists, social workers, or nurses who specialize in trauma and use culturally sensitive techniques

Requires transfer of incarcerated individuals within 7 days to facilities capable of providing proper mental health reentry services if their current facility cannot accommodate their needs

Establishes comprehensive discharge plans including Medicaid enrollment, sufficient medications for transition, and transfer of patient records to post-release health care providers

Provides individuals who received trauma-related clinical intervention with post-release services including mental health care, therapeutic programs (yoga, meditation, physical therapy), family counseling, substance abuse treatment, education, vocational training, housing and job placement information, and money management assistance
