Repeals New York's partially capitated managed long term care (MLTC) program for Medicaid recipients requiring home and community-based services for over 120 days, replacing it with a fee-for-service model effective April 1, 2028

Preserves fully capitated long-term care options including PACE (Program of All-Inclusive Care for the Elderly) for individuals 55+ and MAP (Medicaid Advantage Plus) for individuals 18+ who qualify for nursing home levels of care

Establishes care coordination entities to manage services for enrollees under the new fee-for-service model, with guidelines requiring conflict-free case management to separate assessment functions from service delivery

Requires the Commissioner of Health to convene a stakeholder advisory group including providers, recipients, and direct care workforce representatives to guide the transition and maximize continuity of care while minimizing workforce disruption

Mandates biannual reports on implementation including enrollee satisfaction, service utilization data, enrollment figures, quality metrics, and continuity of care as participants transition from managed long term care to fee-for-service
