Creates a five-year window following the act's effective date for NYSSHP providers in good standing to elect participation in the rate modernization process

Addresses funding disparity where NYSSHP currently provides only $2,964 per year for individuals and $3,900 for families, while ESSHI funds units at $25,000 per unit for both services and operating costs

Responds to findings that one-third of NYSSHP providers have merged, closed, or stopped providing supportive housing services in the last twenty years due to inadequate funding

Directs the Office of Temporary and Disability Assistance (OTDA) to promulgate rules and regulations necessary for implementation
