Requires physicians convicted of sexual offenses or found civilly liable for sexual offenses against patients to provide courts with a list of all medical practices, hospitals, and healthcare facilities where they have worked

Mandates that healthcare facilities notify all current and former patients of the physician via certified mail and email, disclosing the physician's name, license number, conviction or civil liability details, and penalties imposed

Requires the state medical licensing board to create a standardized disclosure form and make it available on its website for healthcare facilities to use

Imposes a $20,000 fine for each 30-day period that medical practices or hospitals fail to send required disclosures to patients

Directs the Department of Education and Department of Health to locate patient records and send disclosures when courts cannot contact a physician's affiliated healthcare facilities

Takes effect 60 days after becoming law
