Expands existing health insurance coverage requirements for enteral formulas to also include infant and baby formulas for home use, whether administered orally or via tube feeding

Requires a written order from a physician or licensed health care provider stating the formula is medically necessary and proven effective as a disease-specific treatment

Covered conditions include inherited diseases of amino acid or organic acid metabolism, Crohn's Disease, gastroesophageal reflux, chronic intestinal pseudo-obstruction, severe food allergies, eosinophilic disorders, and impaired nutrient absorption

Establishes a minimum coverage of $3,000 per calendar year or any continuous 12-month period for infant and baby formulas per insured individual

Applies to individual health insurance policies, group policies, and health maintenance organization contracts that provide prescription drug coverage, effective January 1 following enactment
