Requires the Commissioner of Health to automatically assign Medicaid recipients needing community-based long term care services for more than 120 continuous days to a managed long term care plan if they have not selected one themselves, considering factors like prior care workers, quality performance, and geographic accessibility

Allows Medicaid recipients seeking long term care services to attest to their accumulated resources rather than provide documentation, unless they are applying for long term care for the first time or their resources have increased

Establishes automatic recertification for four categories of Medicaid recipients: managed long term care plan enrollees, managed care plan enrollees receiving personal care services, Aged/Blind/Disabled category recipients with fixed Social Security income, and Medicare Savings Program recipients with fixed Social Security income

Preserves recipients' obligation to report changes in income or other eligibility factors, and guarantees notice and hearing rights if found ineligible during automatic recertification

Takes effect 180 days after becoming law, with provisions requiring federal CMS approval before resource attestation and automatic recertification changes can be implemented
