Requires the Office of Temporary and Disability Assistance to create a single universal application form for all public assistance benefits administered by the office

Local social services districts must provide the universal benefits form to all applicants seeking public assistance

The universal form must include all questions and information necessary to determine eligibility for all applicable public assistance programs

Office must review and update the universal benefits form at least once every six months to incorporate any changes required by law, rule, or regulation

Takes effect 180 days after becoming law, with immediate authorization to develop necessary rules and regulations for implementation
