Establishes a health care disparities data collection system to track health outcomes and care quality disaggregated by race, ethnicity, sex, primary language, disability status, and sexual orientation

Requires health insurers and Article 28 facilities (hospitals, nursing homes) to submit data on subscribers, patients, applicants, and health outcomes to the Department of Health

Mandates public insurance programs report take-up rates and retention rates broken down by demographic factors

Requires the Department of Health to post collected data on a public website within 90 days of receipt, in downloadable format at no charge, disaggregated by county and industry type

Takes effect three months after federal regulations implementing Title XXXI of the Affordable Care Act are issued, or July 1, 2025, whichever is later
