Mandates reporting of statewide waitlist numbers broken down by geographic area, individual's age, caregiver's age, and type of placement needed (certified out-of-home, supervised, supportive, or medically intensive care)

Requires data on placements made during each six-month period, including individuals placed from the waitlist and emergency placements for those not on the waitlist, with reasons provided

Must include information on placement failures or alternative placement requests, along with cost breakdowns, funding sources used (including OPTS and NYS-CARES), and remaining appropriation balances

Requires five-year projections for individuals needing out-of-home residential placement or additional in-home supports, along with a plan for meeting those placement needs
