Expands the definition of home care insurance coverage to include services for transitioning patients from hospitals, nursing facilities, or other institutional settings to their homes

Adds social work, respiratory therapy, and nutritional counseling to the list of covered home care services alongside existing physical, occupational, and speech therapy

Removes the previous 40-visit minimum annual cap on home care visits, allowing insurers to manage benefits through utilization review and preauthorization instead

Requires home care coverage when needed to prevent hospitalization, nursing facility confinement, or other out-of-home services that would otherwise be covered under the insurance contract

Takes effect January 1 following enactment and applies to all policies issued, renewed, modified, or amended on or after that date
