Replaces "fetal death registration" terminology with "pregnancy loss reporting" throughout public health law and expands the definition to include spontaneous miscarriage, stillbirth, and legal terminations at any gestation

Requires pregnancy losses from miscarriage or stillbirth to be electronically reported to the Department of Health within 72 hours; induced terminations only require registration if the individual requests it to facilitate disposition of remains

Removes requirements for identifying information about the pregnant individual from certificates and reports unless specifically requested, and prohibits disclosure except to the individual, their representative, and authorized department personnel

Transfers reporting responsibilities from local registrars to the state Department of Health (or NYC Department of Health and Mental Hygiene in New York City) and requires electronic filing systems

Imposes civil penalties up to $5,000 per violation for releasing information that could identify an individual in connection with a pregnancy loss certificate or report
