Establishes an emergency insulin program to provide emergency supplies of analog insulins to individuals with type 1 diabetes or insulin-dependent type 2 diabetes who are uninsured, underinsured, or at risk of serious health effects due to lack of insulin

Requires pharmacies with 20 or more locations in New York and 340B-eligible healthcare facilities to register as emergency insulin programs by January 1, 2026, with the full program available to recipients by April 1, 2026

Creates a sliding scale cost-sharing system with a maximum cap of $100 for a 30-day supply of analog insulins and related supplies

Establishes an emergency insulin program trust fund to receive appropriations and contributions from analog insulin manufacturers through public-private partnerships

Mandates the commissioner publish reports on program outcomes, including number of recipients served, types and amounts of insulin provided, costs, savings, and partnerships, within one year of enactment and every two years thereafter
