Establishes a grant program for health care quality innovation and improvement through home care, administered by the commissioner with budget director approval

Eligible recipients include certified home health agencies, long term home health care programs, licensed home care services agencies, managed long term care plans, and managed care plans working with contracted home care agencies

Program focus areas include falls prevention, health care disparities, septicemia early identification, cardiovascular health, pressure ulcer prevention, care transitions, and coordination with hospitals, physicians, and behavioral health providers

Funding supports quality measurement tools, electronic records integration, staff training in specialty care and care management, technology-based patient care approaches, and evidence-based practice adoption

Commissioner must establish application and project scoring process in consultation with home care providers, managed care plans, and statewide associations; state grants available if federal funding is unavailable
