Authorizes health insurers, Article 43 corporations, and HMOs to offer incentive or reward programs supporting wellness, preventive care, population health, care coordination, disease management, mental health and substance use disorder treatment, and cost containment initiatives

Caps the total value of incentives at an aggregate average of $600 per year per insured, excluding wellness programs already governed by existing law (section 3239)

Permits reward programs to target specific populations based on age, sex, or health status, and allows time-limited offerings including sweepstakes, raffles, or contests

Creates a new framework for group accident and health insurance policies allowing insurers to adjust premiums based on a range of commission options filed with the superintendent, without violating anti-rebate rules

Requires that for policies subject to guaranteed issuance requirements, the full range of commission options must be made available to all similarly situated groups seeking coverage
