Requires law enforcement agencies to include lethality assessment questions in the standardized domestic incident report form when responding to domestic violence incidents

Mandates the Division of Criminal Justice Services, in consultation with the Office for the Prevention of Domestic Violence, to update the domestic incident report form and develop training by January 1, 2026

Requires all law enforcement officers to complete lethality assessment training by January 1, 2027, with the training incorporated into requirements for new officers thereafter

Specifies 12 lethality assessment questions officers must ask victims, covering topics such as weapon use, strangulation, threats to kill, firearm access, jealousy/control, and stalking behaviors

Requires officers to refer victims to local domestic violence programs and the state hotline, and to file for an extreme risk protection order if assessment results indicate credible risk of serious harm
