Prohibits insurers and HMOs from entering contracts that require including all members of a provider group (medical practices, facilities) in their network, placing them in the same tier, or including them in all products offered

Bans "most-favored-nation" provisions in provider contracts, though insurers and providers may still negotiate rates that are as favorable or more favorable than rates with other entities

Prohibits contract terms that prevent insurers from using benefit designs to encourage members to seek services from higher-value providers

Bans contract clauses that limit disclosure of service fees or allowed amounts to insured individuals or their health care providers

Takes effect January 1, 2026, with any contract clauses violating these provisions becoming null and void while remaining contract terms stay in effect
