Creates the Office of Census Services within the Department of State to promote complete census counts, coordinate outreach efforts, develop partnerships with community organizations, and prepare annual population estimates for state and local governments

Establishes a 30-member New York State Census Counts Commission with appointments by the Governor, legislative leaders, and representation from academic institutions and community organizations to study past census undercounts and develop strategies for accurate counts

Requires the commission to submit reports in the fifth year of each decade (starting 2025) with action plans for the next census, and again in the ninth year (starting 2029) detailing implementation progress and funding needs

Mandates development of a "New York Census Bill of Rights and Responsibilities" affirming rights to participate free from intimidation, maintain confidentiality, choose response method, request language assistance, and verify census worker identity

Creates a new misdemeanor offense for impersonating a census enumerator, punishable by up to one year in jail and/or a fine up to $1,000

Appropriates $15 million to the Department of State to implement the act's provisions
