Mandates public community forums held 150 days before full hospital closures and 90 days before unit closures, with 14-day advance notice and both virtual and in-person accessibility

Requires hospitals to submit health equity impact assessments and closure plans, then revise plans within 30 days after community forums to address stakeholder concerns

Establishes Public Health and Health Planning Council review of closure applications with recommendations to the commissioner, who must approve or deny within 30 days of receiving the recommendation
