Establishes the New York State Public Health Care Option Program, a government-run health insurance plan available to all state residents who choose to enroll

Requires the Commissioner of Health to set income-adjusted premiums with benchmark goals below comparable commercial plans, establish annual cost-sharing limits, and ensure plans qualify for the state Marketplace and federal premium tax credits

Creates an enrollment pathway for employers with fewer than 100 employees, allowing their workers to join the program regardless of residency status

Establishes a 13-member Board of Trustees with appointees from the Governor, legislative leaders, and ex-officio members from the Department of Health and Department of Financial Services to oversee the program

Creates a dedicated fund to hold premiums, federal payments, and other revenues to support program operations, with an implementation plan due within 12 months of enactment
