Mandates all health insurance policies providing medical or comprehensive coverage, as well as Medicaid, to cover patient navigation services for individuals with serious conditions expected to last at least 3 months who are at high risk for hospitalization, nursing home placement, worsening symptoms, physical/mental decline, or death

Patient navigation services include screening for nonclinical and social needs, referrals for transportation, employment, job training, food insecurity, childcare, housing, and language/health literacy support

Coverage extends to assistance with enrollment in government and assistance programs, arranging and accompanying patients to in-person and virtual healthcare visits, and transition of care supports

Reimbursement is limited to individuals who have completed a national patient navigation certification program or other Department of Health-approved training, working under supervision of a licensed physician or healthcare provider

Takes effect January 1 following enactment, applying to all policies and contracts issued, renewed, modified, or amended on or after that date
