Requires New York Medicaid to cover all medically necessary gender-affirming care regardless of whether federal funds are available for such coverage

Establishes unlawful discriminatory practices for health care entities (hospitals, licensed health professionals, insurers) to refuse services or discriminate based on age, race, creed, color, national origin, citizenship/immigration status, sexual orientation, gender identity or expression, military status, sex, disability, genetic characteristics, familial status, marital status, or domestic violence victim status

Prohibits insurers from denying, limiting, or excluding coverage based on sexual orientation, gender identity or expression, or transgender status, and bars designating these characteristics as pre-existing conditions

Mandates that all hospital, surgical, or medical insurance policies cover medically necessary treatments for gender dysphoria or gender incongruence without annual deductibles or coinsurance (except for high deductible health plans which may apply their annual deductible)

Requires insurers to take reasonable steps, including requesting additional information, before denying claims for services typically provided to a different gender or sex than the insured's designated gender
