Adds mandatory electronic personal income tax filings as a new method for New Yorkers to register for the Donate Life Registry for organ, eye, and tissue donation

Amends Public Health Law section 4310 to include tax documents subject to mandatory electronic filing under section 29 of the tax law alongside existing registration options (driver's license, voter registration, health insurance applications, etc.)

Requires the Commissioner of Health to ensure tax forms include space for applicants to register or decline registration, with clear language asking "Would you like to be added to the Donate Life Registry?"

Extends registration availability through this new method to individuals aged 16 and 17 years old

Takes effect 180 days after becoming law, with the Commissioner of Health authorized to begin implementation immediately upon passage
