Extends the annual distribution formula for the general hospital indigent care pool indefinitely beyond 2025, allocating $139.4 million to major public general hospitals and $969.9 million to other eligible general hospitals as Medicaid Disproportionate Share Hospital (DSH) payments

Reduces the aggregate annual reduction amount for non-public general hospitals from $235.4 million to $275.4 million for calendar year 2023 and thereafter, while exempting enhanced safety net hospitals from these reductions

Creates a new "qualified safety net hospital" designation for hospitals meeting specific criteria, including treating at least 45% Medicaid/uninsured patients, having at least 36% Medicaid inpatient discharges, and 28% or less commercially insured patients

Establishes new annual Medicaid rate increases starting April 2026 totaling $275 million: $34.125 million for major public general hospital enhanced safety net hospitals, $228.375 million for other qualified and enhanced safety net hospitals, and $12.5 million for hospitals receiving less funding than in 2018

Authorizes $200 million in annual additional inpatient payments to non-state public general hospitals in cities over one million population, and $100 million annually to state, SUNY, and county-operated public hospitals including those in Westchester, Erie, and Nassau counties
